To be a named person for families with children who are disabled or have complex health needs
To supply families with information regarding all relevant provision available, to promote informed choice
To provide a single point of contact for families and professionals to promote communication and understanding of all concerned
The post holder relates directly with parents/carers using the service.
The post holder will report directly to the Regional Service Manager and gain practice guidance through KIDS Portage network
The post holder will liaise with KIDS staff involved in the provision of services to children and their families
The post holder will liaise with multi-agency colleagues and external agencies involved with the project
Based in North West - Wigan £21,000 F/T 36 hours.
